Laurel Home Health/Hospice (LHH) provides a variety of home healthcare and hospice services. LHH is certified by Medicare and Medicaid, and licensed as a home health agency by the Pennsylvania Department of Health.
Home Health Care
Our team of nurses, therapists, social workers, and aides are available throughout Tioga County and parts of adjacent counties to provide specialized home healthcare as ordered by your physician. Home Health assists patients with recovery, helps them regain independence, and generally improves their quality of life. Nurses are available 24 hours a day, 365 days a year. LHH services include care management; professional nursing; psychiatric nursing; intravenous therapy; wound care and dressing changes, mental health nursing care, personal care by trained home health aides; physical, occupational, and speech therapy; medical social work; and referral to additional community services when needed. Referrals come from area hospitals, nursing homes, and the patient’s physician. Friends, family, and even the patient may call to request home health services, and Laurel Home Health will work with the family physician to receive the necessary referral.
Hospice
Our Hospice program offers medical services and compassionate, supportive care to terminally ill patients and their families, generally in the patient's home. Hospice is available throughout Tioga County and parts of adjacent counties. Hospice services are also provided at the skilled nursing facility, The Green Home. Hospice staff and volunteers are dedicated to helping patients spend their remaining days living life in comfort and with dignity, and to supporting patients' families during and after this difficult time. Laurel Hospice staff work with the patient and his or her physician to create an individualized, comprehensive care plan for each patient, based upon needs. Some of the services offered include nursing care, home health aides, social work, pastoral care, physical therapy, occupational therapy, speech therapy, respite care and bereavement support. In Hospice our nurses are on call 24-hours a day, seven days a week to answer questions, give advise and make home visits as necessary. Hospice patients and their families are never “alone.”
Hospice Volunteer Services
Our trained volunteers provide many different services for patients and their families including providing respite time for the caregivers, reading/ writing letters, running errands, performing light housekeeping, or just sitting and talking with the patient.
